Pay:

$75

• $80 per hour OR $95
• $105 per visit (based on experience) +

Mileage Reimbursement Schedule:

Monday-Friday preferred with significant flexibility; occasional weekend visits as needed Make a meaningful impact—one family at a time. What You'll Do Conduct psychosocial assessments and develop individualized care plans Provide counseling and supportive guidance to patients and their families Facilitate advance care planning and education on advance directives Offer crisis intervention and bereavement support Coordinate with community resources and the interdisciplinary care team Educate patients and families on coping strategies and available support services Maintain timely and accurate documentation in the agency EMR Collaborate with physicians, nurses, and other team members What You'll Bring (Requirements) Active, unrestricted California LCSW license Master's Degree in Social Work (MSW) from a CSWE-accredited program Minimum one (1) year of medical social work experience (home health, hospice, or hospital preferred) Thorough knowledge of Medicare home health guidelines Strong clinical assessment, counseling, and crisis management skills Excellent verbal and written communication skills Valid CA driver's license, reliable transportation, and active auto insurance Current TB screening (within the last 12 months) Current Health Assessment and proof of immunizations/ titers: Hepatitis B (or signed declination), MMR, Varicella, and Tdap.
