Temcula and Surrounding Areas We are seeking a compassionate Per Diem Hospice Medical Social Worker (MSW) to support hospice patients and families by providing psychosocial assessments, counseling, and resource coordination as part of an interdisciplinary team.

Responsibilities:

Complete psychosocial assessments Provide emotional support, counseling, and crisis intervention Assist with advance care planning and community resources Collaborate with nurses, chaplains, and IDT members Document visits accurately and timely per hospice regulations

Qualifications:

MSW from an accredited program Current CA LCSW or ASW Hospice or healthcare experience preferred Valid CA driver's license & reliable transportation

Schedule:

Per diem / as needed Flexible weekday availability; occasional weekends possible
