• 4:30 PM Job Summary We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join a pediatric outpatient team at UCSF Health Benioff Children's Hospital.

The ideal candidate will have strong experience in pediatric outpatient care, case management, psychosocial assessments, and multidisciplinary care coordination. This role requires flexibility to float to other UCSF facilities as needed. Responsibilities Conduct comprehensive biopsychosocial assessments for pediatric patients and families. Develop individualized care plans and provide ongoing case management. Coordinate care with physicians, nurses, therapists, and community agencies. Participate in interdisciplinary care conferences and discharge planning. Connect patients and families with appropriate community resources and protective services. Provide crisis intervention and assess patients for suicide, homicide, self-harm, substance use, and other psychosocial risks. Ensure compliance with NASW Code of Ethics and organizational policies. Document all patient interactions accurately in Epic EMR. Requirements Active California Licensed Clinical Social Worker (LCSW) license. Minimum of 2 years of LCSW experience. Recent pediatric social work experience required. Outpatient clinic experience required. Case management and care coordination experience required. Experience with biopsychosocial assessments and multidisciplinary teams. Proficiency with Epic electronic medical records. Ability to float to other UCSF facilities when needed. Preferred Qualifications Spanish bilingual. Teaching hospital experience. Knowledge of Child Protective Services and community resources. Experience with high-risk patient interventions and behavioral health. Facility Requirements
