The Role We're looking for a compassionate, skilled Licensed Clinical Social Worker (LCSW) who is dedicated to helping individuals build healthier, more stable lives. In this role, you'll provide individual therapy, contribute to integrated treatment planning, and collaborate closely with our medical and administrative teams to ensure each patient receives comprehensive, stigma-free care. What You'll Do Provide individual counseling to patients experiencing substance use disorders, co-occurring conditions, and other mental health concerns Conduct biopsychosocial assessments and develop personalized treatment plans in partnership with patients and the care team Maintain accurate and timely clinical documentation, including progress notes, treatment plan updates, and consents Collaborate closely with nurse practitioners, support staff, and other providers to deliver coordinated, whole-person care Participate in regular case consultations and interdisciplinary team meetings Respond to patient messages and inquiries through secure platforms (e.g., Spruce), offering clinical guidance or routing as needed Assist with crisis intervention and safety planning when appropriate Support continuity of care by helping connect patients with community resources, support groups, or higher levels of care Stay current on evidence-based practices, trauma-informed care, addiction treatment, and emerging therapeutic modalities Contribute to a warm, respectful, and non-judgmental clinic culture for patients and staff What We're Looking For Active LCSW license in Colorado Experience working with substance use disorders and co-occurring mental health conditions Strong clinical assessment, documentation, and treatment-planning skills A patient-centered, trauma-informed therapeutic approach Excellent communication, collaboration, and problem-solving abilities Comfort working in a fast-paced, integrated care environment A team player who is compassionate, adaptable, and committed to dignity-centered care Schedule Monday
