Full Job Description Schedule:
Monday-Thursday 8am-5pm
Pay Range:
$90-95k per year
Essential Duties:
Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.) Experience and willingness to work with a diverse population with a wide range of mental health challenges, including substance abuse. Work effectively and collaboratively with the primary care medical providers to deliver whole person care Coordinate care with other community providers and provide linkage and referrals to other services as needed. Set and monitor achievable clinical goals and outcomes Participation in provider team meetings. Ability to provide both in person and telehealth treatment. Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial) Timely documentation Other duties as assigned •This role will see clients at 4 different offices and travel between offices will be required.
Requirements:
Master's Degree (from accredited university/college) in Social Work License required: LCSW Self-motivated and the ability to work independently as well as within a team Experience coordinating care within the community to wrap around services that may not be available within the primary care setting. Ability to assess and appropriately manage crisis situations that require risk evaluation and management.
