Network, ensuring seamless warm handoffs from EMS and hospital staff to the receiving clinic. This position is essential to maintaining continuity of care and preventing individuals seeking opioid recovery from falling through service gaps.

Qualifications:

Master's Degree in Social Work from a CSWE accredited institution 2 years of experience as a Social Worker in a rural environment preferred Current and unrestricted DOH-issued LCSW Ability to read, speak, and write the English language About Calhoun Liberty Hospital Association, Inc Located in Blountstown, Florida, CLH is a 501(c)(3) nonprofit organization that includes a 10-bed Critical Access Hospital; Calhoun-Liberty Primary Care (RHC) and Calhoun County EMS (911 on-the scene care, emergency transportation an extended transportation needs).
