This role services the Broward County area. This full-time position works weekends; Saturday, Sunday, and Monday, 8 a.m. - 8 p.m (36hrs). In this important clinical support role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care—directly enhancing the quality, dignity, and comfort of their hospice experience. Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness. What You'll Do Provide psychosocial assessment and intervention for patients and families as part of the interdisciplinary team. Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family. Join a team built on kindness, compassion, and Full Life Care for All. What You'll Need Degree in Social Work or a related degree such as Psychology. Masters preferred. One year of experience as a Social Worker. In a hospice setting is preferred. Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
