Pompano Beach, FL Job Details Full-time $150 a week 15 hours ago Qualifications Master's degree in social work CPR Certification First aid Master's degree Driver's License Working with children Full Job Description The Medical Social Worker is responsible for the continuous improvement of the total quality care as it relates to meeting the psychosocial needs of the patient/client. Services are provided in accordance with the patients plan of care, accepted standards of professional practice, applicable law and regulation, as well as the organizations healthcare policies and procedures. Broward Children's Center (BCC) is dedicated to transforming lives by providing specialized care to children and young adults who are medically fragile and developmentally disabled. Our comprehensive services range from 24/7 skilled nursing at our pediatric care facility to developmental education programs, therapeutic support, and family-centered services like respite care, social services, and in-home health support. With unique programs including ABA therapy, developmental preschools, K-12, Home Health and PPEC outpatient care, BCC ensures each individual receives tailored support to thrive. Master's Degree in Social Work with a LCSW or RCSWI- licensed in the State of Florida and a minimum of 1-2 years' experience working with families and children in a healthcare environment. Experience with the Adult Persons with Disability (APD) preferred. Experience with Medicaid and Social Security process preferred.QUALIFICATIONS

Assess each patient's unique condition, combining data from medical diagnoses, personal interviews and professional knowledge to establish accurate picture of overall mental and emotional health. Makes routine visits to assess resident's psychosocial status. Communicate with physician to report resident progress, needs, concerns, and potential and actual issues or concerns. Maintains accurate and timely records including clinical and progress notes according to department policy. Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions. Develops working relationship with leaders in community resource groups, representing the organization at meetings, resource fairs and on various committees that address the social needs of residents such as Developmental Services, etc. Participates in planning and implementing quality improvement activities. Provides positive customer relations in dealing with residents, families, physicians, fellow department employees, other Skilled Nursing Facility departments, referral resources, service agencies, and others. Assist with obtaining school based program components. Adheres to department and SNF policies. Conduct monthly Resident Council Meetings in Group Homes and Skilled Nursing Facility Clear understanding of the Home school or Community MDS / Care Plans / Advance Directive and Guardianship Maintains accurate case notes on services delivered, develops service plans with parents, and tracks and submits monthly billing Assures that the program is in compliance with contractual requirements, utilization of program units, funding, Company policies and procedures and all applicable state and federal laws. Collaborates with Social Services team members from other programs and departments to cross refer clients and ensure clients are benefitting from all relevant BCC programs Assist clients to access appropriate benefits and basic needs; appropriate medical, mental health and substance use treatment providers Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues Assists with the development, implementation, and administration of the Medical Supports Program. Assist with any community inquiries on all agency services, client referrals to community resources, and represent the program at occasional community events and/or presentations Flexibility of working hours will be required to meet the child and family's needs which include evening (2-3 per week) and weekend hours as needed Other duties may be assigned.CERTIFICATES, LICENSES, REGISTRATIONS
