Job Description:
  • Ensures timely communication with patients, their families, and healthcare providers through professional collaboration.
  • Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs.
  • Develops and documents patient-centered plans of care based on assessment, diagnosis, and patient profiles.
  • Evaluates and modifies plans of care based on patient responses and attainment of expected outcomes.
  • Adheres to established standards of practice and the National Association of Social Workers' code of ethics.
  • Provides leadership and support, organizing program materials and promoting care coordination.
  • Assists with community outreach educational programs related to care coordination.
  • Coordinates care across the continuum to ensure safe, efficient, and cost-effective resource utilization.
  • Identifies patient populations needing care coordination and addresses their health-related needs, including social determinants of health.
  • Maintains an active caseload of "at risk" patients and optimizes their independence through education and community resources.
  • Provides crisis intervention and community support resources for patients and families experiencing significant stress due to illness.
  • Other duties as assigned.
Knowledge, Skills, and Abilities:
  • Computer skills and experience with Microsoft Office programs, including Word, Excel, Outlook, and PowerPoint are required.
  • Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health related organizations, various external parties, and regulatory agencies.
  • Ability to use office equipment such as telephone, personal computer, copier, fax machine, etc. [Required]
  • Complex Ambulatory care experience [Preferred]
  • Population health experience [Preferred]
