Location/Setting:

Cumming, GA (no local candidates within 50 miles) Responsibilities Conduct psychosocial assessments of patients and families to identify barriers to care, discharge needs, and psychosocial risk. Coordinate patient care with the interdisciplinary team to support timely disposition and safe transitions. Provide counseling and linkage to resources for coping with illness, treatment decisions, and family communication. Develop and implement comprehensive care plans; document in the electronic medical record; ensure compliance with policies and regulations. Participate in care conferences and discharge planning meetings; advocate for patient-centered planning.
