Pay Range:

$25/hr - $26/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

Weekly Gross - $2,036.80 (Based on completion of 40 hours a week) Per-Diem - Inclusive of Meals, Incidentals & Lodging $996.80 / week Shift Details;

Shift:

12:00 PM - 8:30

PM Schedule:

Monday through Friday Conduct comprehensive psychosocial assessments for patients and families Develop and coordinate discharge planning and continuity of care Provide emotional support, counseling, and crisis intervention services Collaborate with physicians, nurses, case managers, and interdisciplinary teams Assist patients with community resources, placement needs, and post-acute care planning Maintain accurate and timely documentation in accordance with hospital policies Ensure compliance with patient confidentiality and healthcare regulations

Key Requirements and Technology Experience:

Minimum 2 years of recent experience within Social Work specialty Active Georgia LCSW license required Recent acute hospital experience required Strong communication, organizational, and patient advocacy skills Ability to work effectively in a multidisciplinary healthcare setting Experience in hospital discharge planning and care coordination Familiarity with EMR systems and hospital workflows Ability to manage high patient volumes in acute care settings Our client is a leading Healthcare Industry, and we are currently interviewing to fill this and other similar contract positions.
