• Hospice Full-Time | Flexible Schedule Seasons Care Hospice is currently seeking a compassionate and experienced Licensed Medical Social Worker (LMSW) to join our Hospice team .

This role is essential in supporting patients and families through end-of-life care by providing emotional support, counseling, and resource coordination in alignment with hospice philosophy and regulations.

Travel:

Must be willing to travel to assigned service areas / communities (Marietta GA)

Pay Range:

$32 to $38 per hour Key Responsibilities Conduct psychosocial assessments for hospice patients and families Provide emotional support, counseling, and crisis intervention as needed Assist patients and families with advance directives, care planning, and end-of-life decision-making Coordinate community resources, financial assistance, and support services Collaborate with the interdisciplinary hospice team to develop and implement patient care plans Participate in care conferences, documentation, and care coordination Ensure compliance with hospice regulations, state, and federal guidelines Provide bereavement support and referrals as appropriate Qualifications Active Licensed Medical Social Worker (LMSW) license in the state Hospice experience preferred (medical, home health, or palliative care experience strongly considered) Strong knowledge of hospice philosophy and interdisciplinary care Excellent communication, organizational, and documentation skills Compassionate, patient-centered approach to care Ability to work independently and manage time effectively Reliable transportation and willingness to travel between communities Why Join Us?
