About Us:

We are seeking a dedicated and empathetic Medical LCSW to join our interdisciplinary team and support patients and families through complex medical journeys.

Medical LCSW Key Responsibilities:

Provide psychosocial assessments and counseling to patients and families in medical settings (inpatient, outpatient, or home health). Collaborate with physicians, nurses, and case managers to develop and implement care plans. Assist with discharge planning, resource coordination, and crisis intervention. Advocate for patients' mental health and social needs within the healthcare system. Maintain accurate and timely documentation in the EHR.

Medical LCSW Qualifications:

Master's degree in Social Work (MSW) from an accredited program. Current Hawaii LCSW license in good standing. Minimum 2 years of experience in a medical or hospital setting preferred.
