Medical Social Worker

River Falls is a dedicated home health agency committed to providing compassionate, patient-centered care. We are seeking a Medical Social Worker to join our team to support patients and their families with the psychosocial and medical challenges associated with home-based care.

Job Summary:

The Medical Social Worker will conduct psychosocial assessments, provide counseling, assist with care coordination, and connect patients and families to community resources. This role is essential in helping patients successfully manage their health and maintain independence in their homes.

Responsibilities:

Conduct comprehensive psychosocial assessments of home health patients and their families Develop and implement individualized care plans addressing medical, emotional, and social needs Provide counseling and emotional support to patients and caregivers Assist with discharge planning and transitions of care Connect patients with community resources such as financial aid, transportation, and support services Advocate for patient rights and facilitate communication between patients, families, and healthcare providers Collaborate closely with nurses, therapists, physicians, and other team members Maintain detailed and timely documentation of all social work activities

Qualifications:

Master's Degree in Social Work (MSW) from an accredited program Current state licensure as Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Experience in home health, medical social work, or community-based social services preferred Strong assessment, counseling, and case management skills Excellent communication and interpersonal abilities Ability to work independently and as part of a multidisciplinary team Valid driver's license and reliable transportation Pay 33-38$ an hour or if you prefer per visit 55$
