Medical Social Worker (Master's License Level) Emotional support. Resource guidance. Advocacy. Join The Care Team Hospice and make a meaningful difference in the lives of patients and families throughout the Crown Point, IN area. We're seeking a compassionate Part-Time/PRN Social Worker to provide emotional support, care coordination, and valuable resources to patients navigating their hospice journey. If you're passionate about serving others and want the flexibility of a part-time/PRN schedule while working with a supportive team, we'd love to connect with you.
