POSITION SUMMARY
The Licensed Clinical Social Worker (LCSW) performs comprehensive intake assessments, including mental status and psychosocial evaluations, and uses this information to develop and evaluate individual service plans. Offering individual, family, and group counseling, they apply approved treatment approaches and execute therapeutic interventions, skill building, and case management for clients. The LCSW maintains meticulous chart documentation, ensuring compliance with federal and state regulations, and prepares patient progress reports. In times of mental health crisis, they provide support, serving as an information resource for mental health issues. Additionally, they collaborate with physicians, therapists, and other medical professionals as needed.
Essential Duties :
1. Performs intake assessments, which include the completion of the initial mental status assessment, and psychosocial assessment. 2. Plans and evaluates individual service plans and monitors plan objectives. 3. Performs individual, family, and group counseling utilizing approved treatment approaches as sanctioned by Shalom. 4. Prepares patient progress reports and completion notices. 5. Executes therapeutic interventions, skill building, and case management for clients. 6. Completes chart documentation reviews that include the psychosocial history, treatment planning, chart progress notes, quarterly reviews, and discharge summaries. 7. Maintains accurate records to ensure compliance with all federal and state regulations. 8. Assists in mental health crisis situations.
Education & Qualification Requirements :
Master's degree in social work Licensed Clinical Social Worker At least two (2) years of counseling experience. Previous experience using the DSM-5 and the ability to assess mental health disorders.
Knowledge, Skill, Ability and Work Difficulty :
Must possess and/or demonstrate the following: o Expertise in diagnostic formulation as it relates to the DSM-5; o Good interpersonal abilities with a tactful approach; o Excellent organizational skills; o Support of rehabilitative concepts; o Ability to take direction in various settings; o Ability to manage multiple priorities simultaneously; o Capability to work with diverse populations; and o Ability to collaborate well with others.
