Bridge Clinic Patient Population:
Adults 18+
Location:
Cambridge, MA Pay:
Up to $3,200/week
Responsibilities:
Provide behavioral health and psychosocial assessments Support patients with substance use disorders and dual diagnoses Develop and coordinate patient care plans Collaborate with interdisciplinary healthcare teams Maintain accurate documentation in
EPIC EMR
Provide crisis intervention and patient advocacy Assist with discharge planning and community resources
Requirements:
