Major Responsibilities:
1. Provides clinical service to patients and their families through individual, family, or group work, such as biopsychosocial assessment, crises intervention, treatment planning and counseling relative to medical conditions. 2. Identifies clinical resources and make referrals when necessary. 3. Collaborates with physicians, care coordinators, discharge planners, and other health care professionals to identify barriers to discharge or treatment and help resolve these issues with patients, families, or external sources. 4. Advocates for patients and families as they negotiate the health care system so that effective care, treatment, and planning may occur. 5. Documents all information obtained in evaluation, assessment, consultation, referral, and disposition to comply with regulatory requirements, as well as ensuring best possible patient outcome. 6. License/Certification/Education:
Required:
1. Master's Degree in Social Work. 2. Current Massachusetts Licensed Clinical Social Worker (LCSW) licensure.Experience/Skills:
Required:
1. Demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, ability in discharge planning, and ability to work with a multidisciplinary team. 2. Data compilation and analysis skills. 3. Excellent written and communications skills. 4. Basic computer skills.Preferred:
1. One to three years of Social Work experience. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents. III.Physical Demands and Environmental Conditions:
Position requires work indoors most often in an office environment. However, work may also include monitoring patients' movements and activities resulting in the need for rapid physical response to unsafe patient movements. This includes standing, walking, running, reaching, bending, lifting, and stooping.Job Purpose:
Provide consultative clinical social work services in a healthcare setting, including biopsychosocial assessment, crisis intervention, counseling, discharge planning, and coordination of community resources to support safe transitions of care.Key Responsibilities:
- Perform comprehensive biopsychosocial assessments (including risk/safety assessment as applicable) and develop intervention plans aligned with patient goals and the care team plan.
- Manage complex psychosocial cases, provide crisis intervention, brief counseling/therapeutic interventions, and psychoeducation to patients and family.
- Identify barriers to treatment/discharge (e.g., housing, safety, financial, caregiver support, transportation) and coordinate plans to address them to progress through the care continuum.
- Coordinate referrals and linkage to community resources and/or state agencies across the care continuum (shelter/housing supports, Social Determinants of Health (SDOH) benefits, behavioral health, Substance Use Disorders (SUD) treatment, etc.).
- Advocate for patients and families as they navigate the healthcare system; facilitate problem‑solving and conflict resolution when needed.
- Collaborate in Multi Disclipnary Rounds (MDR) and/or multi-disciplinary discussions; communicate plans, barriers, and disposition needs clearly and timely.
- Document assessments, plans, interventions, referrals, and outcomes in the Electronic Health Record (EHR) to meet organizational, payer, and regulatory requirements.
- Liaison between medical team and Office of General Counsel (OGC) to mitigate legal issues (e.g. guardianship /affirmation /expansions).
- Provide oversight to Social Worker (LCSW) staff, if applicable.
- Complete billing entry for clinical services, if applicable.
- Support basic data tracking/compilation and quality improvement initiatives as assigned.
