! Medical Social Worker Opening at a Hospital in Annapolis, MD. Full time, Direct Hire, M-F and 1 weekend a month or weekends available

Position Qualifications:

MD LMSW or higher Inpatient Social Work experience Discharge planning/case management experience required EPIC experience •FULL TIME, ONSITE, Day shift- 8 or 10 hour shifts, Weekends available including Monday or Friday (3) 12 hour shifts, DIRECT HIRE Yearly (Based on Experience): $73,000 to $85,000

Responsibilities:

Early Identification, high risk screening, and readmission screening of all patients.

Services include:

adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments and coordination with the multi-disciplinary team.
