Job Description
Seasons Home Health Care is a clinician owned, home health care agency. We strive to provide the highest quality of care and customer service in a patient focused environment and are looking for additions to our team that can fulfill our mission. We are seeking fun and energetic people to add to our team! We are looking to add a part-time or PRN Medical Social Worker to our team! The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee. A Medical Social Worker is responsible for the following:
- Plans/coordinates all social services within Residence.
- Participates in the development of the plan of care.
- Participates in patient care conferences.
- Maintains current knowledge of Medicare Conditions of Participation for social work services.
- Develops a cooperative relationship and communicates effectively and professionally with physicians.
- Participates in the coordination of All Seasons' services with the services of other community resources; uses community resources.
- Serves as a resource person to employees, patients, physicians and other allied healthcare providers.
- Attend meetings, CEUs on a regular basis
