Home Health Job Summary:
We are seeking a compassionate and experienced Medical Social Worker (MSW) to provide home health services, supporting patients and families in navigating healthcare and social needs.
Key Responsibilities:
Assess patients' social, emotional, and financial needs to create care plans. Provide counseling and connect patients with community resources and support services. Collaborate with interdisciplinary teams to ensure continuity of care. Advocate for patients' needs with payers, providers, and other entities. Maintain compliance with Medicare and Medicaid documentation requirements.
