Job Description

The Care Team Hospice is seeking a compassionate and dedicated Part-Time Hospice Medical Social Worker to join our team! This is a rewarding opportunity for a social worker who is passionate about supporting patients and families through life's most challenging moments while making a meaningful impact in end-of-life care. As a Hospice Medical Social Worker, you will serve as a vital member of the interdisciplinary team, providing emotional support, psychosocial assessments, counseling, bereavement services, and community resource coordination. You'll work closely with patients, families, caregivers, and clinical team members to ensure each patient receives compassionate, holistic care that honors their goals, wishes, and quality of life. Success in this role is measured by your ability to build trusting relationships, advocate for patients and families, provide meaningful support during times of crisis and transition, and connect individuals with the resources they need. You'll play an essential role in helping patients and loved ones navigate the hospice journey with dignity, comfort, and peace.

Company Match Additional Voluntary Coverage Key Responsibilities:
Complete psychosocial, emotional, spiritual, and bereavement assessments while developing and supporting individualized plans of care. Provide counseling, crisis intervention, emotional support, and advocacy for hospice patients and their families. Collaborate with the interdisciplinary team to identify patient and family needs, coordinate services, and support quality end-of-life care. Connect patients and families with community resources and support services while promoting continuity of care. Maintain timely and accurate documentation, participate in care planning meetings, and support bereavement and family services as needed.
Qualifications:
Master's Degree in Social Work (MSW) preferred; Bachelor's Degree in Social Work or related field considered in accordance with state regulations. Previous hospice, home health, healthcare, or counseling experience preferred. Strong assessment, counseling, communication, and advocacy skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Valid driver's license, reliable transportation, and current automobile insurance required.
