Job Summary •This position may be eligible for a hiring incentive up to $2,500 as well as relocation reimbursement •The Community Behavioral Health Hospital (CBHH) in Alexandria is seeking a compassionate, person-centered Licensed Social Worker to join our team dedicated to make a difference in the lives of others. In this key role, you will deliver integrated health care and social services, along with participating as an active member of the interdisciplinary team. Operated by the Minnesota Department of Human Services, the CBHH is a small (16-bed) inpatient psychiatric facility for adults experiencing acute mental health episodes. The length of stay is usually less than 60 days, during which treatment focuses on rapid assessment, stabilization, patient education, and discharge planning involving community support resources, with the overall goal of preventing future episode.
Responsibilities include:
Meet one-on-one with patients weekly to provide support, psychoeducation, address barriers to discharge, and care planning Facilitate patient and family education Initiates appropriate referrals for post-discharge follow-up services Maintains appropriate and effective relationships with community providers Qualifications Minimum Qualifications Bachelor's degree in psychology, social work, behavior analysis, or related field; and Licensure/Certification in the State of Minnesota as a Licensed Social Worker (LSW) Two years of post-bachelor's degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below: Delivering psychoeducation and/or behavioral interventions Providing case coordination among multidisciplinary team members Conducting comprehensive assessments Leading development of treatment/support plans as required by program licensure Planning discharge/transition and/or aftercare OR Master's degree in psychology, social work, behavior analysis, or related field; Licensure/Certification in the State of Minnesota Licensed Graduate Social Worker (LGSW), Licensed Independent Social Worker (LISW), or a Licensed Independent Clinical Social Worker (LISCW) One year of post master's degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below: Delivering psychoeducation and/or behavioral interventions Providing case coordination among multidisciplinary team members Conducting comprehensive assessments Leading development of treatment/support plans as required by program licensure Planning discharge/transition and/or aftercare OR Master's degree in psychology, social work, behavior analysis, or related field; and Independent Licensure/Certification in the State of Minnesota Licensed Independent Social Worker (LISW), or a Licensed Independent Clinical Social Worker (LISCW) Additional Requirements To facilitate proper crediting, please ensure that your resume clearly describes your experience in the areas listed and indicates the beginning and ending month and year for each job held.
