32 guaranteed Position Summary We are seeking an experienced Licensed Clinical Social Worker (LCSW) for a 12‑week travel assignment in a hospital setting. The social worker will provide support to patients and families, assist with discharge planning, and coordinate community services. Candidates must have at least 2 years of hospital social work experience and hold an active Minnesota LSW license. Requirements Minimum 2+ years of hospital social work experience (required) Experience with discharge planning (required) Experience working with all patient populations (required) Active Minnesota LSW license (required at submission) Epic EMR experience (preferred) Must provide work history, skills checklist (within 1 year), and 2 supervisory references Responsibilities Provide psychosocial support to patients and families adjusting to illness, injury, or hospitalization Assist healthcare teams in understanding social, economic, and emotional factors affecting patient care Facilitate timely and appropriate discharge planning and coordination with next level of care Identify and coordinate community services for patients in need Document care and interventions accurately in EMR Work every 6th weekend as scheduled Work Environment Hospital setting with interdisciplinary support (PT, pharmacy, radiology, social services, rapid response teams) Patient population: adults, geriatrics, pediatrics, neonates, and infants Dress code: business casual Charting system: Epic Compliance requirements: MN DHS Net Study & fingerprinting required for all patient‑facing positions
