PRN Job Summary Provides psychosocial assessment, counseling, advocacy, and coordination of community resources for patients and families. Supports continuity of care, discharge planning, and emotional well-being in the hospital setting. Minimum Qualifications Bachelor's degree in Social Work required Licensed or license-eligible per state requirements Basic Life Support (BLS) certification Knowledge of community resources and discharge planning Preferred Qualifications Hospital or healthcare social work experience preferred Essential Duties & Responsibilities Conduct comprehensive psychosocial assessments for patients and families Provide counseling, crisis intervention, and emotional support Coordinate discharge planning and post-acute community resources Collaborate with physicians, nurses, and interdisciplinary care teams Advocate for patient rights and continuity of care Maintain accurate, timely clinical documentation Physical Requirements Ability to stand and/or walk for extended periods. Frequent bending, reaching, squatting, and kneeling. Lift, carry, push, and pull up to 50 lbs independently; >50 lbs with assistance/devices. Assist with patient repositioning and transfers using proper body mechanics and equipment. Manual dexterity for clinical procedures and computer use; adequate vision, hearing, and speech. All candidates must be able to perform the essential functions of this position.
