Henderson, NV Job Details Contract $30 - $40 an hour 1 day ago Qualifications Teamwork Stress management Licensed Clinical Social Worker Outpatient experience Patient interaction Full Job Description Heart Well Rehabilitation is a growing outpatient rehabilitation clinic in Henderson, Nevada, providing Cardiac Rehabilitation, Pulmonary Rehabilitation, and Physical Therapy services. We are committed to delivering compassionate, patient-centered care through a collaborative, multidisciplinary approach. We are seeking a compassionate and motivated Licensed Clinical Social Worker (LCSW) to join our team on a part-time or per diem (as-needed) basis. This position offers the opportunity to support patients recovering from cardiac and pulmonary conditions by addressing the emotional, behavioral, and psychosocial factors that influence successful rehabilitation. Responsibilities Conduct psychosocial assessments for patients participating in rehabilitation programs. Administer and document PHQ-9 depression screenings and other psychosocial assessments, as appropriate. Identify barriers to treatment participation and develop strategies to improve patient engagement. Provide brief counseling, emotional support, stress management techniques, and motivational interviewing. Educate patients and families on lifestyle modification, coping strategies, and available community resources. Collaborate with physicians, nurses, physical therapists, respiratory therapists, and other healthcare professionals to support individualized patient care plans. Coordinate referrals to behavioral health providers, community organizations, and support services when appropriate. Maintain timely, accurate clinical documentation in accordance with clinic policies and applicable healthcare regulations. Participate in interdisciplinary team meetings and contribute to quality patient care. Qualifications Current Nevada Licensed Clinical Social Worker (LCSW) license required. Strong communication, counseling, and interpersonal skills. Ability to work effectively in a multidisciplinary healthcare environment. Experience in cardiac rehabilitation, pulmonary rehabilitation, outpatient rehabilitation, hospital, or other medical settings is preferred. Experience with psychosocial assessments, PHQ-9 screening, motivational interviewing, and clinical documentation is highly desirable.
