Key Responsibilities:
Conduct comprehensive psychosocial assessments to identify client needs, strengths, and housing barriers. Develop individualized service and housing plans in collaboration with clients. Assist clients in securing transitional and permanent housing, including completing applications and coordinating with housing providers. Provide ongoing case management, including regular meetings to monitor progress and adjust goals. Connect clients to community resources such as mental health services, healthcare, employment programs, and financial assistance. Advocate on behalf of clients with landlords, service providers, and government agencies. Facilitate life skills training, including budgeting, tenant rights, and independent living skills. Maintain accurate and timely documentation in compliance with agency and funding requirements. Participate in multidisciplinary team meetings and coordinate care with internal and external partners. Support crisis intervention and stabilization as needed.
Qualifications:
Master's Degree in Social Work (MSW) from an accredited program.
Pay:
$65,000.00
