One Weekend Per Month Salary:
$87,658 - $131,487 annually

Comprehensive Medical, Dental, Vision, PTO, Retirement Plan, and More Position Overview We are seeking a compassionate and experienced Social Work Care Manager to join our multidisciplinary care team. In this role, you will coordinate patient-centered care throughout hospitalization, facilitate safe and timely discharges, and help reduce avoidable readmissions by addressing psychosocial, financial, and community resource needs. Key Responsibilities Conduct comprehensive psychosocial assessments for patients upon admission. Identify high-risk patients and develop individualized care transition plans. Coordinate discharge planning and post-acute care services. Educate patients, families, and caregivers regarding treatment plans and community resources. Collaborate with physicians, nurses, therapists, and external agencies to ensure seamless care transitions. Address barriers to discharge, including social, financial, and environmental challenges. Facilitate referrals to rehabilitation facilities, home care agencies, hospice, behavioral health services, and community programs. Participate in multidisciplinary rounds and care conferences. Ensure compliance with regulatory, payer, and hospital requirements. Maintain accurate and timely documentation in the electronic medical record. Support initiatives focused on length-of-stay reduction, readmission prevention, and patient satisfaction. Qualifications Master's Degree in Social Work (MSW) from an accredited institution. Current New York State LMSW or LCSW license required.
NYS SCREEN
Certification required within the first 3 months of employment. Minimum 2 years of hospital-based, acute care, care management, discharge planning, or related healthcare experience preferred. Strong knowledge of community resources, healthcare systems, and transitional care planning. Excellent communication, assessment, and problem-solving skills.
