Job Description

Functioning within the Health System's mission, values, objectives, policies and procedures, the Clinical, Master's prepared (MSW), Social Worker manages patients with complex psychosocial and financial crises. This includes offering assistance and follow through with eligibility determination for social programs and funding sources, as well as qualifying patients for community assistance from a variety of special funds and agencies. This individual works closely with other care managers and social workers in the community who provide this assistance. The Oncology Social Worker II anticipates needs throughout the continuum of care and makes recommendations to providers and Navigators. The Oncology Social Worker II receives referrals from interdisciplinary team members to include physicians, advance practice providers, nurse navigators, staff nurses and other members of the care team. This may include calling in additional resources such as pain management, palliative care, hospice, protective custody, guardianship, financial assistance, spiritual support as well as assisting with the preparation of end-of-life decision documentation. The Oncology Social Worker II works closely with patients and their support systems to ensure an understanding of the resources available, helps both the patient and support system adjust to the illness at hand and provides realistic expectations of future management of the illness.
Education:
Graduate of an accredited Social Work degree program with a MSW required.
Licensure/Certification:
Must have a current valid Social Worker license in the State of Ohio.
Experience:
Three (3) years of previous experience in an acute care setting is preferred.
Skills:
Must possess strong interpersonal and communication skills, be motivated and self-directed, and have the ability to integrate numerous job responsibilities.
