Job Description

Make a Meaningful Difference Every Day Are you a compassionate Social Worker who is passionate about supporting patients and families through life's most important moments? Join our hospice team and become part of a mission-driven organization dedicated to providing exceptional end-of-life care with dignity, comfort, and respect. We are seeking a Medical Social Worker to serve patients in Newark, Ohio and surrounding communities. This is a field-based position that combines patient visits in homes and facilities with collaboration among an experienced interdisciplinary care team.
Schedule:
Monday-Friday, 8:30 AM-5:00 PM Weekend and holiday rotation required. This postiion requires travel between patients homes and facilities located inNewark, Ohio and surrounding areas. What You'll Do As a Hospice Medical Social Worker, you'll play a vital role in helping patients and families navigate the emotional, social, spiritual, and practical challenges that accompany serious illness and end-of-life care.
Key responsibilities include:
Conduct comprehensive psychosocial assessments for hospice patients and families Provide counseling, emotional support, and crisis intervention as needed Assist families with advance care planning and community resource coordination Address cultural, spiritual, and psychosocial needs unique to each patient and family Support patients and loved ones through anticipatory grief and bereavement processes Collaborate with nurses, physicians, chaplains, and other members of the hospice care team Maintain accurate and timely clinical documentation Advocate for patient and family needs while promoting quality of life and comfort What We're Looking For Required Qualifications Bachelor's degree in Social Work Ohio Social Work license (LSW or LISW) Minimum of one year of social work experience in a healthcare setting Valid driver's license, reliable transportation, and acceptable driving record State-required automobile insurance coverage Strong computer and documentation skills Ability to travel throughout the service area, including during inclement weather Preferred Qualifications Master's degree in Social Work (MSW) Hospice or palliative care experience CHP-SW or ACHP-SW certification CPR certification Notary designation or willingness to obtain within six months of hire Why Join Us?
