Job Description
The Clinical Social Worker position provides biopsychosocial assessments/interventions, crisis assessment/interventions, care coordination, referral coordination, and advocacy for patients and their support system(s) as a member of the interdisciplinary team for Lower Columbia Hospice & Palliative Care. This position oversees the bereavement program to support loved ones of hospice patients as well as providing grief support to the community through education and support groups, often in partnership with trained volunteers.
Job Requirements Knowledge of/Skill/Ability to:
Maintains the standards of Social Work excellence in accordance with principles set forth by the State Health department, Columbia Memorial Hospital, and agency policies and procedures. Performs short-term crisis intervention and individual or family counseling when indicated. License/Certification- An Oregon Licensed Clinical Social Worker (LCSW) license is required, or ability to transfer licensure from another state with 60 days of hire.
- Ability to maintain credentialing and appropriate documentation for insurance companies, if needed.
- Valid driver's license with current vehicle insurance coverage and safe driving record is required.
