Full time Medical Social Worker hybrid in person/remote
Qualifications:
Education/credentials in Master of Social Work, Associate Social Work license, or LCSW 1 year of experience in social work, medical social work, or related field (field experience during grad school acceptable) In depth understanding of the implications of illness, home health, hospice and the dynamics of grief Demonstrates working knowledge of psychosocial interventions, community resources and social service programs Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non- medical people Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures Ability to work both independently and collaboratively as part of an interdisciplinary team with nurses, chaplains, physical therapists, occupational therapists, speech therapists, among other providers. must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR) Bilingual preferred (Cantonese or Mandarin) Must be willing to work in East Bay where patients are located
Job Summary:
Join Asian Network Pacific Home Care and Hospice in supporting and serving a unique and diverse population with compassionate and dignified care. The ideal candidate will be a full time Medical Social Worker serving East Bay area. This candidate will be an integral member of a multidisciplinary team. The candidate will collaborate with patients to identify, monitor, and collaborate on short term and long term goals to support their medical crisis and/or end-of-life journey. This position comes with a competitive salary, flexible schedule, hybrid model, and hours with clinical supervision to work towards your licensure. The population includes working with patients in home health undergoing a medical crisis and in hospice facing end-of-life issues This position will require on site patient visits in patient homes or facilities. Telehealth visits to caregivers may be required. Meetings and documentation can be done remotely. Candidate is an integral member of a multidisciplinary team consisting of doctors, nurses, physical therapists, occupational therapists, speech therapists, chaplains, and CNAs Competence and comfort with compassionate lens working with families in medical crisis, older adults, the disabled and/or the terminally ill Assesses psychosocial needs of patients/families, and develops plan of care to assist patients and families/caregivers to cope with short term and long term goals, including but not limited to psycho-social issues, social and caregiving support/assets, bereavement risk and support, among others Implements and knowledge of psychosocial interventions, including crisis intervention, suicide and risk assessment, grief/bereavement counseling, problem solving, stress management and developing healthy coping strategies Assesses patient's financial and social assets and eligibility for community resources, complete resource linkage and make referrals as needed, advocate for patients to receive appropriate social services Collaborates with patients, families, and team members to identify goals and plan of care that will help patients and families restore their psychosocial functioning. Monitor and implement goals and track progress Gains confidence and cooperation from the patient and family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication Provides education to help patients and families access appropriate services such as caregiver resources, advanced care planning, funeral arrangements, financial and insurance resources, long term facility placement, and needs related to end-of-life issues Coordinates/consults exchange of information between patient, family, clinical staff, and community providers/resources.
