Job Description

About Us We are a small nonprofit organization dedicated to advocating for and supporting individuals and families with disabilities. We are seeking a compassionate, flexible, and proactive individual to provide case management, family support, and mental health services to our clients. Training and ongoing support will be provided. Position Summary The Case Manager/Social Worker will assist individuals and families by coordinating services, conducting assessments, providing wraparound support, and connecting clients with community resources. This role requires strong communication skills, organization, and a passion for serving families with diverse needs. Responsibilities Research and develop innovative ways to recruit clients for case management services Assist with coordinating client services alongside the LCSW and agency staff Facilitate and support case management services for individuals and families Maintain and update client files and documentation Contact clients and families to coordinate care, education, support groups, and emergency financial assistance Research community resources, including Medicaid, food assistance, and other support services, and provide referrals Coordinate services between clients and staff to ensure continuity of care Assist with support group meetings and coordination of client services Conduct regular client check-ins to monitor well-being and ensure services are meeting family needs Provide client education regarding health, wellness, and available resources (training provided) Thoroughly document all client interactions and provide weekly reports of family contacts Attend and help coordinate client activities, support groups, camps, holiday events, and health fairs Assist clients with emergency financial assistance applications Provide wraparound services and case management support to the entire family Provide written participation reports for activities and events Conduct intake assessments and health checks Diagnosis and provide mental health support and counseling services as appropriate Qualifications Current Texas Social Work License required (LMSW or LCSW) Previous case management and/or social work experience required Experience working in nonprofit organizations or with high-needs families preferred Strong organizational skills and attention to detail Experience facilitating support groups, client services, or customer service preferred Proficiency with Microsoft Office, Excel, Google Docs, email, and internet research Compassionate, professional, and proactive attitude Bilingual Spanish/English preferred License/Certification Social Work License (LMSW or LCSW) - Required Benefits Flexible schedule Meaningful work supporting individuals and families in the community Training and professional support provided
