Job Description

Mountain Valley Child and Family Services (MVCFS) provides individualized treatment for youth and teens, ages 10-21 in residential treatment in Nevada City and Sacramento, and TBS services in Nevada City, Sacramento, and the Bay Area. Community-based services coordinators (CB coordinators) are responsible for providing documentation and case management for the MVCFS rehabilitative specialty mental health services (SMHS) program. CB coordinators will ensure quality service delivery and maintain the highest standards of treatment and supervision. CB coordinators are responsible for meeting organizational requirements and contractual obligations for SMHS.
Qualifications License:
LMFT, LCSW, LPCC
Experience in treatment of severely emotionally disturbed children and adolescents Compliant with California Department of Social Services childcare employment standards Required screening: CA Dept. of Justice Live-scan, FBI and Child Abuse Clearance Index (CACI) background checks Physical examination to ensure ability to perform necessary job duties Drug screen Tuberculosis test
Knowledge and Skills:

Essential Duties and Responsibilities:
Service delivery Liaise with caregivers, counties and providers as necessary to ensure effective coordination of treatment plans. Provide direct-service to assigned clients when required Leadership Oversee a team of direct-service providers, ensuring daily operations reflect the agency's mission, equity goals, and trauma-informed values; independently identify and immediately remediate any cultural shifts, personnel friction, or ethical lapses to protect employee morale and service quality. Role model and encourage professional communication between program staff to ensure effective team collaboration and seamless service delivery; Proactively resolve communication gaps to foster a culture of mutual respect, equity, and psychological safety, and to ensure all team members feel valued and supported. Management Coordinate with clinical supervisors and other program leadership regarding ICC, TBS and IHBS referrals, coach assignments and case management. Oversee the completion of paperwork for ICC, TBS and IHBS cases (this includes FAB/TX plans, monthly reports, discharge reports and progress notes. Liaise with clinical supervisor regarding SMHS program requirements Conduct weekly group supervision meetings and individual supervision meetings as needed Provide staff training on TBS, IHBS, and ICC policies, procedures and service provision Professionalism Attend other meetings as requested Follow clinical supervisor instructions and guidance in providing individual case coordination with

coaches, IHBS staff and CFTs Work cooperatively and respectfully with all clients, co-workers, caregivers and providers Communicate an attitude of respect and service Be responsive to the requests of clients, families, counties, providers and workers Strictly maintain client confidentiality Business (productivity, contractual compliance ensure service providers meet weekly productivity targets ensure service-provider progress notes are completed and submitted within MVCFS specified timeframes Operations Maintain quality assurance: Audit client paperwork Track and complete review of progress notes and ensure compliance to county deadlines Track and monitor assigned service-provider productivity Ensure assigned service-provider county training compliance Maintain client/coach roster for TBS and IHBS Maintain tracking procedure for monthly reports and authorizations Supervise completion of the initial TBS coach and IHBS staff training process Contact providers, caregivers and clients as needed to check in about progress and ensure satisfaction with services Be responsible for the maintenance and safety of all client records. Regulatory Ensure compliance with applicable regulatory and contractual requirements, and local, state and federal laws Professional development Attend MVCFS and county sponsored relevant SMHS trainings Other duties as assigned CB coordinators may be assigned other duties to ensure effective service delivery and oversight Lines of supervision Clinical/administrative supervision: Community-based (CB) services coordinators report directly to the clinical program manager. This includes regular clinical consultation, performance evaluations, and oversight of service delivery standards.
Direct Reports:
CB services coordinators provide direct supervision to the following roles: Therapeutic behavioral services (TBS) providers Intensive home-based services (IHBS) providers Intensive Care Coordination (ICC) providers Other assigned direct service staff General Requirements Always engage in safe practices Read and follow all MVCFS safety manuals Use equipment, supplies and vehicles in a safe and appropriate manner Strictly adhere to traffic laws and follow safe driving practices Maintain environmental safety, including but not limited to safety procedures with regard to lakes and streams, extreme weather, hazardous chemicals, natural disasters, etc. Immediately report suspected or observed unsafe practices to a supervisor or director. Know and follow policies and procedures Read and follow all relevant MVCFS policy and procedure Immediately report violation of policy or procedure to a supervisor or director. Adhere to standards and expectations Attend and actively participate in all required staff meetings, team briefings, one-on-one supervision meetings and performance evaluations, and participate in company surveys Complete all mandatory training within designated timeframes, including but not limited to compliance, safety, and role-specific skill development Adhere to all organizational standards of conduct, including confidentiality, professional ethics, and workplace behavior Communicate effectively and professionally with supervisors, colleagues, clients/customers, and external partners Support a respectful, inclusive, ethical and safe work environment in alignment with company values and policies Demonstrate consistent and reliable attendance and punctuality; follow established procedures for requesting time off and reporting absences Utilize the timekeeping system to track all work hours in real time Maintain all required licenses, certifications, and clearances in good standing Ensure compliance with applicable regulatory and contractual requirements, and local, state and federal laws

Working Environment:
CB services specialists typically operate in various indoor and outdoor community environments and drive their personal vehicles to meet with clients and families. typically operate in an office environment, outdoors, a clinical setting, remote work setting, etc. operate in an environment where the noise level is generally low may be exposed to standard office equipment, varying weather conditions, household chemicals, patient care environments, etc. may be exposed to children with propensities for self-endangering behaviors and violence and aggression towards others. In some circumstances CB services coordinators may be asked to provide telehealth services.
Provision of telehealth services requires:
written authorization by the payor and the CB services coordinator's direct supervisor preapproval of the site from which services will be conducted strict adherence to 12503 MVCFS Technology-Based Service Delivery Policy .
Physical Requirements:
While fulfilling the duties of this job, CB specialists may be required to: sit, stand, walk, bend, and reach for extended periods. use hands to operate vehicles and frequently use a computer and telephone. occasionally climb, stoop, kneel, or crouch depending on task requirements.
Work schedule and travel:
CB coordinators normally work a forty-hour week. However, this position regularly demands substantially more hours. CB coordinators are scheduled to work primarily during the optimal times for providing staff supervision and program oversight and direction. Travel may be required for meetings, training, or work-related tasks.
Salary and Benefits:
The CEO of MVCFS determines the salary for this position within the limits of the existing range established by the board of directors, based on an evaluation of the applicant's qualifications. This position receives the benefits accorded all full-time professional staff. Conditions of Employment Employees must remain in good standing with all regulatory requirements and all MVCFS standards, and must pass comprehensive criminal background checks and drug screenings. Employment begins with a 90-day trial period. This is an opportunity for MVCFS to evaluate the employee's performance. It also is an opportunity for employees to decide whether they want continued employment with
MVCFS. MVCFS
may extend the introductory period at its discretion. Completion of the introductory period does not alter at-will employment status.
Pay:
$80,000.00 - $88,000.00 per year
