Responsible for the delivery of services to the Healthy Start client population as outlined in the Healthy Start guidelines. The Healthy Start Social Worker facilitates services across a continuum through collaboration with community resources, the Healthy Start team, and other professional disciplines to ensure effective and efficient delivery of Healthy Start services and the achievement of desired client outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the delivery of services rendered. Contributes to the success of the Healthy Start team through excellence in customer service and performance improvement techniques that demonstrate positive outcomes of said services. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start Team and are expected in this role.
Requirements Education:
Bachelors degree in social work required.
Experience:
1 year of social work experience in medical or healthcare setting preferred.
