Social Worker Description

• Coordinate support services such as transportation, housing assistance, or financial aid for patients and families navigating the healthcare system.
• Facilitate communication between healthcare providers, patients, and families to ensure understanding of care plans and available resources.
• Maintain documentation and case records to support continuity of care and compliance with institutional policies.
• Advocate for patient rights and access by working with insurance providers, legal aid, and community organizations.
• Develop and manage resource directories and referral networks to connect patients with external support programs and social services. • Requires a minimum of a Master's degree in Social Work, Clinical Psychology, or Psychology. A concentration in Gerontology, Pediatric, Public Health, Substance Abuse, Mental Health or other related fields is preferred
