Clinical Social Worker - LCSW / LICSW (Travel Assignment)

Hyannis, MA Duration:
26 Weeks (Contract-to-Hire)Shift:
Day Shift | 8:00 AM - 4:00PM Schedule:
40 Hours/Week | Rotating Weekends & Holidays Job Summary A healthcare facility is seeking an experienced Clinical Social Worker (LCSW or LICSW) to support patients and families in an acute care hospital setting. The role involves psychosocial assessments, crisis intervention, discharge planning, and coordination of community resources to ensure safe patient care and transitions. Responsibilities Conduct psychosocial assessments for patients and families Provide crisis intervention and emotional support Assist with discharge planning and care coordination Collaborate with physicians, nurses, case managers, and community agencies Coordinate referrals for rehabilitation, counseling, home care, and community services Support patients with behavioral health, substance abuse, and social service needs Maintain accurate documentation in EHR systems Advocate for patients and families throughout treatment and discharge Requirements Master's Degree in Social Work (MSW) Active LCSW or LICSW license required CPI and/or MAB certification required within 30 days of hire Strong communication and organizational skills Ability to work in a fast-paced hospital environment Proficient with electronic documentation systems Preferred Experience Acute care hospital social work experience Crisis intervention or behavioral health experience Discharge planning and community resource coordination Experience working with interdisciplinary healthcare teams For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
