Job Description

Job Overview We are seeking a compassionate and dedicated Hospice Medical Social Worker to join our interdisciplinary care team. In this vital role, you will provide emotional support, counseling, and resource coordination to patients and their families facing life-limiting illnesses. Your expertise will help enhance quality of life through comprehensive psychosocial assessments, individualized care planning, and therapeutic interventions. Responsibilities Conduct thorough psychosocial assessments of patients and families to identify emotional, social, and behavioral needs. Develop personalized care plans that incorporate counseling, resource linkage, and support strategies aligned with hospice care standards. Provide individual, family, and group therapy sessions utilizing clinical counseling techniques such as motivational interviewing, cognitive behavioral therapy, and crisis intervention. Collaborate with medical teams to coordinate patient care, ensuring compliance with HIPAA regulations and accurate medical documentation. Assist with intake procedures, including evaluating patient eligibility and facilitating smooth transitions into hospice services. Maintain detailed medical records and documentation within electronic medical record (EMR) systems, ensuring accuracy and confidentiality. Offer specialized support for patients with complex conditions such as dementia, Alzheimer's disease, PTSD, autism spectrum disorders, or developmental disabilities. Qualifications Bachelor's or Master's degree in Social Work from an accredited program; licensure or certification as required by state law. Proven experience in hospice care or related fields such as hospital or inpatient settings. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and healthcare regulations including HIPAA. Experience working with diverse populations including children, seniors, individuals with disabilities or mental health conditions. Proficiency in EMR systems and data collection methods for clinical research or quality improvement initiatives. Skilled in crisis management, behavioral health interventions, case management, and diagnostic evaluation. Excellent communication skills for engaging patients and families through compassionate counseling and psychotherapy techniques. $35.00 - $40.00 per hour
