Job Description

Company Overview Friends of Caroline Hospice is a dedicated non-profit organization that has been serving the community since 1980. As the first hospice in the area, our mission is to provide compassionate, quality-of-life care that offers hope and encouragement to individuals nearing the end of their life journey, along with support for their families, friends, and the broader community. Job Summary We are seeking a compassionate and dedicated Hospice Social Worker to join our team. The ideal candidate will possess strong interpersonal skills and a deep understanding of end-of-life care, providing emotional support, counseling, and resource coordination for patients and their families. As a Hospice Social Worker, you will play a vital role in enhancing quality of life through comprehensive psychosocial assessments, advocacy, and personalized care planning. Responsibilities Conduct thorough psychosocial assessments to identify patient and family needs, preferences, and goals. Provide emotional support and counseling to patients facing life-limiting illnesses and their loved ones. Assist with navigating healthcare systems, insurance processes, and community resources to ensure comprehensive support. Collaborate with interdisciplinary team members to develop individualized care plans that address social, emotional, and practical needs. Advocate for patient rights and preferences while ensuring dignity and respect are maintained throughout care. Offer guidance on advance directives, end-of-life planning, and grief support services. Document all interactions accurately in accordance with organizational policies and regulatory standards. Skills Proven experience as a social worker within hospice or palliative care settings. Strong understanding of psychosocial assessment tools and counseling techniques. Excellent communication skills with the ability to build trust and rapport with diverse populations. Knowledge of community resources, healthcare systems, and legal considerations related to end-of-life care. Compassionate demeanor combined with resilience to handle emotionally challenging situations. Ability to work collaboratively within an interdisciplinary team environment. Certification or licensure as a Licensed Clinical Social Worker (LCSW) or equivalent is preferred but not mandatory.
