At Cascades at Riverwalk , we are committed to providing quality care that supports each resident's physical, emotional, and psychosocial well-being. We are a people-first organization guided by integrity, teamwork, and meaningful outcomes. Position Summary The LCSW oversees social service support for residents in our skilled nursing facility. This role works closely with residents, families, care teams, and community resources to ensure residents receive the support and advocacy they need. Responsibilities Advocate for resident rights and individual needs Communicate with residents and families regarding care updates, goals, and discharge planning Coordinate referrals and services with outside agencies and community resources Assist residents with financial, legal, and supportive service resources Build positive relationships between residents, families, and staff Educate team members on resident-centered care and psychosocial support Participate in interdisciplinary care plan meetings and develop appropriate plans of care Address and assist with resident or family concerns and grievances Complete social service assessments and discharge planning documentation Maintain accurate and timely documentation in the electronic medical record system Supervise and support Social Services staff as needed Benefits Medical, dental, and vision insurance 401(k) with employer match Up to 16 days of paid time off annually Company-paid life insurance Qualifications Bachelor's degree in Social Work or Human Services required; Master's degree preferred LCSW strongly preferred Minimum of one year of supervised social work experience in a healthcare setting 1 year experience in Long Term Care Experience with electronic medical records required Strong communication and organizational skills Commitment to supporting the senior population If you are passionate about advocating for residents and supporting families through every stage of care, we encourage you to apply.
