We are currently seeking a qualified and experienced PRN Hospice Medical Social Worker (MSW) to support our Hospice program as patient admissions occur. Compensation $60.00 per patient visit / assessment Mileage reimbursement provided in accordance with Evergreen Health Care Management's mileage reimbursement policy $25.00 per required meeting PRN / Per Diem position with compensation based on services performed and organizational needs Position Summary The PRN Hospice Medical Social Worker will provide psychosocial services to hospice patients and their families on an as-needed basis. The ideal candidate must have prior Hospice or Home Health experience and be able to independently conduct and document Initial Psychosocial Assessments within the EMR system. This position is designed for experienced professionals seeking flexible work opportunities while making a meaningful impact on patients facing serious illness and end-of-life care needs. Essential Duties and Responsibilities Conduct Initial Psychosocial Assessments for newly admitted hospice patients. Evaluate emotional, social, financial, environmental, and family support needs. Develop and implement psychosocial care plans as part of the interdisciplinary care team. Provide counseling, education, and emotional support to patients and family members. Assist patients and families in accessing community resources and support services. Support patients and families with end-of-life planning, adjustment, grief, and bereavement-related concerns. Collaborate with physicians, nurses, chaplains, and other members of the interdisciplinary team. Maintain accurate, timely, and compliant documentation within the EMR system. Participate in interdisciplinary team meetings as requested. Ensure compliance with Medicare Hospice Conditions of Participation and company policies. Required Qualifications Master's Degree in Social Work (MSW) from an accredited institution. Active Virginia LMSW or LCSW license. Bilingual communication skills required. Previous Hospice or Home Health experience. Ability to independently complete and document an Initial Psychosocial Assessment without supervision. Strong verbal, written, and interpersonal communication skills. Reliable transportation and willingness to travel throughout Northern Virginia. Preferred Qualifications Experience working with diverse cultural and linguistic populations. Familiarity with hospice documentation standards and EMR systems. Knowledge of Medicare Hospice regulations and interdisciplinary care planning. Schedule PRN / As Needed Flexible scheduling based on patient admissions and hospice census.
