Day shift, 4x10 days Start date: 05/18/2026 Travel assignment in a Level II trauma center setting with two campuses Responsibilities Conduct discharge planning and care coordination for adult and geriatric inpatients Perform interdisciplinary collaboration with PT, interpretation services, pharmacy, social services, and transportation teams Complete social work documentation in the charting system (EPIC) in a timely, accurate manner Support high-risk patients with Medicaid/Medicare discharge placements and DME referrals as appropriate Manage a caseload with typical daily census patterns and adapt to weekend fluctuations Provide case management support across medically complex inpatient populations Requirements Active WA license in hand; pending license accepted per posting (confirm status at submittal)

Certifications:

BLS Licensure options for MSW track include

LICSW, LSWAIC

(associate), LASW, LSWAA Minimum 1 year of acute care and discharge planning / care coordination experience; first-timers accepted Ability to work in a hospital inpatient setting and float as needed across campuses within the area Benefits and Support Competitive weekly pay provided per week 70 hours guaranteed per 2-week pay period
