Day 5x8-Hour (08:00 - 16:30)

Duration:

AL01 Job Summary A Licensed Clinical Social Worker (LCSW) provides mental health assessment, diagnosis, treatment, counseling, and case management services to individuals, families, and groups. The LCSW works collaboratively with interdisciplinary healthcare teams to develop treatment plans, provide crisis intervention, coordinate care, and support patients in achieving their mental health and psychosocial goals.

Responsibilities Duties:

Conduct comprehensive psychosocial and mental health assessments. Diagnose mental health conditions within the scope of licensure. Develop individualized treatment and care plans. Provide individual, family, and group psychotherapy. Conduct crisis intervention, suicide risk assessments, and safety planning. Coordinate discharge planning and continuity of care. Connect patients with community resources and support services. Collaborate with physicians, nurses, case managers, and other healthcare professionals. Maintain accurate, timely, and HIPAA-compliant clinical documentation. Monitor patient progress and modify treatment plans as needed. Advocate for patients and families to ensure appropriate care and resources. Educate patients and families on treatment options and coping strategies Participate in interdisciplinary meetings, case reviews, and quality improvement initiatives.
