Full Job Description Job Title:
Licensed Clinical Social Worker (LCSW)
Location:
Seattle, WA 98122
Schedule:
Day Shift

Weeks Compensation:
FROM $1,800 per week (Local Candidates) FROM $2,200 per week (Traveler Candidates)
Key Responsibilities:
Conduct psychosocial assessments and develop care plans for acute care patients. Coordinate discharge planning, including arranging community resources and post-acute care services. Provide crisis intervention, case management, and collaborate with interdisciplinary teams. Assist with advance care planning, protective services, and patient advocacy. Maintain accurate and comprehensive documentation. Ensure continuity of care through referrals and appropriate resource linkage.
Required Licenses & Certifications:
Active state licensure (LSWAIC or equivalent) BLS certification (AHA)
Education & Experience:
