July 13, 2026 Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments for hospitalized patients.
Develop individualized care and discharge plans based on patient and family needs. Coordinate post-discharge services, including home health, rehabilitation, long-term care, and durable medical equipment (DME). Collaborate with physicians, nurses, therapists, case managers, and community agencies to ensure continuity of care. Provide crisis intervention and emotional support to patients and families. Assist with advance directives, guardianship, power of attorney, and protective services referrals as needed. Connect patients with community resources, including Medicaid, Medicare, housing assistance, transportation, medication assistance, and financial support programs. Document assessments, interventions, and care plans accurately in Epic EMR. Participate in interdisciplinary rounds and care conferences. Float to affiliated campuses when required based on patient census. Qualifications
  • Active LCSW or LSWAIC license.
Current BLS Certification. Minimum 1 year of inpatient or acute care hospital experience. Previous experience in case management and discharge planning. Proficiency with Epic Electronic Medical Record (EMR). Excellent communication, critical thinking, and organizational skills. First-time travelers are welcome to apply. HL01
