13 Weeks Position Overview We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join a hospital-based Case Management team in Seattle, Washington. This position focuses on supporting inpatient acute care patients through care coordination, discharge planning, psychosocial assessments, and collaboration with interdisciplinary healthcare teams. The ideal candidate will have prior inpatient or acute care experience and be comfortable working within a specialty hospital environment that supports neuro and cardiac patient populations. Key Responsibilities Provide psychosocial assessments and support for hospitalized patients and families Coordinate discharge planning and transitions of care Collaborate with physicians, nurses, case managers, and interdisciplinary teams Assist patients with community resources, referrals, and support services Support care coordination for neuro and cardiac patient populations Maintain accurate and timely documentation in the electronic medical record system Participate in patient advocacy and care planning activities Float to additional campuses as needed based on census and operational requirements Requirements Active LSWAIC or equivalent Washington State licensure required Minimum 1 year of inpatient or acute care social work experience required Case Management experience required Epic EMR experience required Current BLS Certification required Strong communication, crisis intervention, and care coordination skills Ability to work effectively in a multidisciplinary healthcare environment For more details reach at or Call / Text at 732 791 4807•EXT 4807 .
