Job Description

Position Title:
Licensed Independent Clinical Social Worker (LICSW)
Position Type:
Full-Time, Part-Time, or Per Diem Duties:
Conduct comprehensive biopsychosocial assessments of patients and families, including evaluation of emotional, social, and environmental needs. Develop, implement, and independently manage individualized clinical social work treatment plans in accordance with hospice standards and insurance guidelines, provided in patients' homes or care facilities. Provide billable clinical social work services within the scope of licensure. Collaborate with the interdisciplinary team, including the primary nurse, to establish, evaluate, and modify patient-centered plans of care. Provide advanced clinical interventions addressing psychosocial needs of hospice patients and their families, including crisis intervention, counseling, and end-of-life support. Perform bereavement risk assessments and deliver ongoing grief counseling through both in-person and telehealth modalities. Provide end-of-life and post-death bereavement services, including anticipatory grief counseling and follow-up care for families. Utilize clinical judgment to assess, diagnose (if applicable per licensure), and treat mental health and emotional disorders within scope of practice. Identify and coordinate appropriate community resources and referrals to support patient and family needs. Participate in and lead community outreach and education initiatives related to grief, loss, and hospice care. Provide post-discharge support services as clinically indicated. Maintain accurate, timely, and compliant clinical documentation consistent with regulatory and reimbursement standards. Represent and uphold the philosophy, mission, and services of Hospice of Huntington, Inc.
Position Qualifications:
Master's Degree in Social Work (MSW) required. Current Licensed Independent Clinical Social Worker (LICSW) licensure in West Virginia (and Ohio, if applicable). Minimum of 2 years of post-licensure clinical experience in a healthcare setting; hospice or home health experience strongly preferred. Demonstrated expertise in clinical assessment, diagnosis, and treatment of mental health conditions, grief, and bereavement. Strong knowledge of end-of-life care, family systems, and interpersonal dynamics. Proven ability to provide independent clinical counseling and crisis intervention. Excellent communication, documentation, and interdisciplinary collaboration skills. Valid driver's license and access to a reliable, insured vehicle. Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
