Department Name Community Care Clinic Position Summary Individual, Group, and Family mental health services are rendered across the lifespan by a Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Psychologist as an outpatient behavioral health service under the direction of the Associate Dean of Interprofessional Education and Community Engagement. A person in this position will work closely with patients to assess their mental health needs, create individualized treatment plans, and provide ongoing counseling and support. As part of the Community Care Clinic team, you will collaborate with healthcare professionals to ensure a holistic approach to patient care.

We focus on whole person care, integrating mental health support with physical health resources to foster overall well-being. Job Duties and Responsibilities 1. Provide outpatient behavioral health services in the Community Care Clinic (CCC). 2. Provide individual, group, and family therapy to a diverse client population. 3. Conduct mental health assessments, diagnosis, and treatment planning. 4. Provide coordinated referral plans for clients that the provider lacks the capacity to provide medically necessary outpatient behavioral health services. 5. Create and maintain accurate and confidential patient records in compliance with clinic policies and applicable regulations. 6. Participate in CCC Quality Assurance / Quality Improvement programs. 7. Collaborate with providers across disciplines to enhance Team Based Care and Interprofessional Education. 8. Provide education and mentorship to students in the CCC. 9. Stay up to date on best practices, evidence-based approaches, and relevant mental health trends. 10. Attend staff meetings and participate in clinic-wide initiatives and projects. 11. Maintain licensure and continuing education requirements. 12. Other duties as assigned. Skills Considered Essential for Success

Required Education and/or Experience 1. Be a Social Worker, Professional Counselor, or Psychologist in good standing. 2. Maintain a Quality Assurance Plan 3. Ability to handle sensitive and confidential information with professionalism. 4. Excellent communication and interpersonal skills. 5. Empathy and commitment to providing compassionate care to all patients. 6. Ability to work both independently and collaboratively as part of a healthcare team.
