Wellness Care Clinics is a local outpatient mental health clinic in Mesa, Arizona, providing compassionate, structured support for individuals working toward recovery and improved mental wellness. Our approach centers on compassionate guidance, practical treatment plans, and consistent follow-up to help patients build lasting change. Our team includes licensed mental health professionals and qualified clinical staff who are committed to providing ethical, patient-centered care in a supportive outpatient setting. Position Overview Wellness Care Clinics is seeking a full-time Licensed Professional Counselor (LPC) to provide therapy services for our Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) . The LPC Therapist will work with individuals experiencing mental health and substance use concerns, providing individual counseling, group therapy, assessments, relapse prevention support, recovery skill building, family support sessions, and outpatient follow-up care. The ideal candidate is compassionate, organized, clinically skilled, and committed to helping clients develop practical tools for long-term recovery and emotional stability. Key Responsibilities Provide individual and group counseling for clients participating in IOP/PHP outpatient services. Conduct mental health assessments and assist in developing individualized treatment plans. Support clients with substance abuse and co-occurring mental health concerns. Facilitate relapse prevention planning, including identifying triggers, developing coping strategies, and creating practical recovery action plans. Lead recovery skill-building sessions focused on daily routines, communication, decision-making, emotional regulation, and stability. Provide family support sessions to improve understanding, healthy boundaries, and support systems. Monitor client progress and provide outpatient follow-up care through ongoing check-ins and treatment plan updates. Maintain accurate, timely, and confidential clinical documentation in accordance with clinic policies and professional standards. Collaborate with clinical staff and other qualified team members to coordinate patient care. Maintain professional, ethical, and compassionate communication with clients, families, and staff. Follow all applicable state, federal, and clinic guidelines related to behavioral health services and patient confidentiality. Qualifications Active Licensed Professional Counselor (LPC) license in the State of Arizona. Experience providing mental health counseling in an outpatient, IOP, PHP, substance abuse, or behavioral health setting preferred. Experience facilitating individual and group therapy. Knowledge of substance abuse treatment, relapse prevention, co-occurring disorders, and recovery-focused care. Strong assessment, treatment planning, documentation, and communication skills. Ability to work collaboratively with a multidisciplinary clinical team. Commitment to compassionate, ethical, and patient-centered care. Preferred Skills Experience working with clients in recovery or dual-diagnosis treatment. Familiarity with outpatient behavioral health programs, including IOP and PHP levels of care. Ability to provide practical, skills-based interventions that support long-term wellness. Strong organizational skills and attention to documentation requirements.
