$35,360.00-$39,520.00 per year Position Overview Northland Therapy Services, Inc. is continuing to grow and is recruiting for a Family Support Specialist (FSS) to provide services on the Fort Apache Reservation. The FSS works closely with families, primarily in their homes, to build trusting relationships, support healthy parent-child interactions, and help families create safe, stable, and nurturing home environments. Key Responsibilities
  • Initiate and maintain regular contact with enrolled families through home visits conducted weekly, bi-monthly, monthly, or quarterly based on assigned point levels.
  • Establish supportive, trusting relationships with parents and caregivers while respecting family strengths, individual goals, cultural diversity, and safety concerns.
  • Structure, strategize, and plan home visits that support each family's goals, with ongoing attention to child abuse and neglect prevention, attachment and bonding, and child development.
  • Assist parents in strengthening the parent-child relationship and building skills that contribute to a safe, stable, and nurturing home environment.
  • Provide prenatal education, health education, and information that promotes infant and child health, safety, and development.
  • Collaborate with families to develop meaningful goal plans that reflect their needs, priorities, and strengths.
  • Assist families with scheduling, preparing for, and attending health care appointments and other community agency visits.
  • Conduct the initial 15-question screening of referred families and maintain a detailed contact and referral log.
  • Conduct in-home FROG Assessments in accordance with current Healthy Families America survey standards.
  • Write and score Parent Survey Assessment narratives based on assessment conversations to help determine family eligibility.
  • Review all scored Parent Survey Assessment narratives with the supervisor during supervision within six business days.
  • Maintain all documentation and files, including EOMs, home visit notes, assessments, forms, and related records, according to the NTS Site Policy and Procedure.
  • Keep a detailed, up-to-date calendar of work activities to serve as a time sheet for NTS payroll.
  • Attend regularly scheduled weekly supervision sessions, typically lasting two hours, and seek additional support during the week as needed.
  • Attend monthly team meetings unless prior permission to be absent has been approved.
  • Keep current on training requirements from HFAz Central Administration.
  • Participate in marketing Healthy Families services in identified service areas as appropriate based on caseload, available time, current participant needs, supervisor input, and FAW input.
  • In collaboration with the team, schedule, coordinate, and attend parent groups, activities, and family events.
  • Assist with additional activities outside the primary scope of work when needed to support and benefit the team. Qualifications
  • Minimum of a high school diploma or equivalent.
  • Experience working with or providing services to children and families.
  • Ability to establish trusting relationships with families.
  • Acceptance of individual differences.
  • Experience and humility to work respectfully with culturally diverse families.
  • Knowledge of infant and child development.
  • Willingness to engage in building reflective capacity, including introspection, awareness of self in relation to others, and recognition of the value of supervision.
  • Fingerprint Clearance Card.
  • Background check.
  • Reliable transportation.
  • Current Arizona Driver's License.
  • Automobile liability insurance.
  • Infant Mental Health Endorsement preferred.
